<Name Change>

Please submit a document which has your old and new name, date of birth, and your current (new) address.
-Valid identification documents are as follows (Documents must be valid or issued within six months):

• Photocopy of your Residence Card
• Original copy of your Resident Certificate (Juminhyo)
• Original copy of your Special Permanent Resident Certificate
• Photocopy of your Individual Number Card (front only)
• Photocopy of your Japanese Driver's License (front and back)

<For those who wish to change their registered signature>

• Please register your signature with your new name. Please upload the photo data of your new signature written on a blank piece of white paper.
  - If written on anything other than a blank piece of white paper, the procedure will not be completed.
  - If you need to register your personal seal(Hanko), please contact us via our Web consultation form.
  - If you do not upload a new signature, the previously registered signature/seal will continue to be used as your registered signature/seal with our bank.

• The document must be valid, and the image must be clear and legible.
• The file types that can be uploaded are jpg, jpeg, and png (up to 5MB per file).
