Required Identification Documents for opening PowerFlex account

Please provide and below as your identification documents.

- Customers who classify as U.S. citizens under U.S. taxation laws will be required to provide additional documents. Please click here.

- Please check your name and address on your ID are match the one on your application form. If they don't match, you will be asked to resubmit your application form.

Please provide a photocopy of your "Residence Card" or "Special Permanent Resident Certificate".

Documents	Note
Residence Card (Photocopy)	Please check that the address stated on the card is your current address. The name spelling and address on the account application form must be the same as on your certificate or card (i.e. your nickname will not be accepted). Please photocopy both sides. If your Individual Number is on the back, the back side is not required. The photocopy must be clear and legible. Name, address, date of birth, and card number must be clear and legible. If the "Date of Permission" is not indicated on the card, you will also need a copy of your passport showing that you have been in Japan for at least six months.
Special Permanent Resident Certificate (Photocopy)	Please check that the address stated on the card is your current address. The name spelling and address on the account application form must be the same as on your certificate or card. Please photocopy both sides. If your Individual Number is on the back, the back side is not required. The photocopy must be clear and legible. Please ensure the number on the upper right of the front side is clear. Name, address, date of birth, and card number must be clear and legible.

The photocopy must be clear and legible.
Please ensure the number on the upper right of the front side is clear.
Name, address, date of birth, and card number must be clear and legible.

Please provide "the original copy of Residence Certificate" .

An original copy of Residence Certificate (Issued within the past 6 months)	Please provide all pages, including the page with issue date and seal of the issuer. Please prepare the original certificate for the account holder. If the certificate should include information regarding the household please include those pages as well. Please issue a certificate without your Individual Number (My Number) and your permanent address (Honseki-chi). We need to verify your current address on the certificate. If the certificate should have your Individual Number (My Number) and your permanent address (Honseki-chi) printed on it, please black it out before sending.

TAX Residency Confirmation in FATCA and CRS

Customers who classify as U.S. citizens under U.S. taxation laws will be required to provide both "W-9" and a "Self-Certification of Account Holders Based on the Common Reporting Standard (CRS) and the Act on Special Provisions of the Income Tax Act, the Corporation Tax Act and the Local Tax Act Incidental to Enforcement of Tax Treaties".
